I suppose the program could patch the Shell when it loads to force NTSC or PAL. I don't know if all models of Playstation can output video in either mode. I think some might only output black and white video or no video at all. Though if you're using RGB I guess that would probably work.
Since it's just the Shell/boot up animation that will be in the native region I don't see much point in forcing it to another video mode as whatever software you are loading up will set it's own video mode anyway. Chances are your display supports the console's native video mode anyway or else you wouldn't have it. And again we're talking about a boot sequence for maybe 20 seconds before the video mode changes anyway.
Unless you're meaning some option to start in 60hz (ntsc) mode and force the loaded software into 60hz NTSC too? That would be possible I supposed but I think there are discs out there already that can be used to force a video mode.

The main reason I want my console to boot in 60Hz is because I use a modern display with an external scaler.
This external scaler doesn't like it when the refresh rate of the video signal changes, very rarely it even starts to interpret 59.3Hz (an off-spec NTSC signal from an early PAL model) as 66Hz (I'm guessing it samples the sync signal too soon after the change in refresh rate, or maybe the signal is noisy because my cables suck). This is of course a flaw in the scaler, making the console boot in 60Hz would be a workaround for this.
Another thing is that all of my other consoles output 60Hz, and for some reason my display takes a lot of time to switch from 60 to 50Hz. Often times I can't even see the startup. But I'd like to be able to see it, for a more authentic feel.
I figured Stealth Unlocker's goal is to add functionality without changing the user experience. But I'm not sure a feature like this would fit within the scope of the project.

If so, I'd love to have an option for that
I've heard some later BIOS versions have just one bit to determine if the console should boot with 50 or with 60Hz video output, but I don't think this is the case for older BIOS version. If I remember correctly there was a mod chip that flipped this bit. But that doesn't work on older consoles.
